Position Summary
The Patient Coordinator is responsible for full life-cycle coordination of care and serves as a liaison between rural trauma patients and healthcare system. This position helps manage patient needs, coordinates services across multiple care teams and facilities, and ensures appropriate data collection for the Division for improving rural health outcomes. This position answers all patient questions and collaborates with members of an interdisciplinary team to coordinate care and facilitate resource and discharge planning to reach optimal outcomes for the patient.

Responsibilities
Essential Functions[MM1]

Schedules patient appointments, tests and procedures and ensures the coordination of patient care plans across interdisciplinary teams.
Receives, responds to, and directs patient communications, including phone calls, emails, and other interactions.
Assesses escalated patient questions and ensures the coordination of care plans.
Monitors for completion of outcome surveys to provide team with data to assess outcomes
Identifies patient needs, answers questions and educates new patients on the process so they can make informed decisions.
Identifies resources locally for patients and assists with communication with local providers
Triage referrals from physicians, both internal and external, in addition to self-referred patients.
Gathers needed medical information such as records, slides, films, and ancillary tests to assist in the coordination of patient needs.
Ensures insurance coverage, resolves any issues with coverage, and escalates complicated issues to appropriate financial counselor.
Acts as a liaison with other care groups such as social work, financial counseling, and nutrition to facilitate the patient intake process.
Provides detailed patient instructions and education in collaboration with providers and other clinical staff.
Works with discharge planner to facilitate and align patient needs with resources and/or equipment to meet quality and clinical outcomes.
Assists in the coordination of seminars and community outreach activities.
Monitors program performance and coordinates any programmatic development within the team.
Coordinates treatment planning conferences, logistics, presentations, and associated CME requirements.

QualificationsRequired

Two years applicable background experience in nursing, social work or care coordination.
Bachelor's degree in nursing is preferred. Candidates with a degree in Social Work, Case Management or a related healthcare/human services field will also be considered. Relevant professional experience in care coordination, patient advocacy, case management, or healthcare services is highly valued.

Preferred

Competency in electronic patient charting and scheduling.
Experience in a high volume clinical setting or multidisciplinary clinics or working with high-acuity, complex patients.

Minimum Qualifications
Bachelor's degree or equivalency (one year of education can be substituted for two years of related work experience) in public health, health education, health promotion, management, behavior science, social science, social work or nursing with 2 years relevant work experience or Medical Assistant with State of Utah, National Certification, or equivalency and 5 years of clinical experience.

Demonstrated ability to lead as well as function effectively as a member of a team and successfully promote change. Strong analytical, organization and leadership skills. PC/Windows literate with working knowledge of the following Microsoft applications: Outlook, Word and Excel. Demonstrated human relation and effective communication skills.

This is a patient-sensitve position.

Preferred Experience

Working knowledge of quality improvement and project management methodologies, tools and techniques and principles of change management. Demonstrated ability to effectively facilitate and mentor project teams preferably using QI methods (i.e., PDSA). Familiarity with electronic medical record (EMR) applications.
