Job Summary

As a member of this team, you will play a vital role specific to the Behavioral Health Collaborative Care program. BHCC is an integrated care model which aims to improve behavioral healthcare access for patients with depression, anxiety and other common mental health diagnoses. This program emphasizes mental health as an important component of overall health and wellness and is an integral part of the primary care medical home. Our collaborative care model is a joint effort between the Ambulatory Care Social Work Department and the Department of Psychiatry. The focus is on tracking a patient population with mental health concerns such as depression and anxiety.

Responsibilities*
You will be responsible for:
  • Reviewing records, diagnostic assessment, symptom monitoring, care management and care coordination, evaluating and recording progress with respect to treatment goals, and ongoing collaboration with the patient's medical home team while drawing on social work experience and ongoing assessment of patient needs.
  • Participating in weekly psychiatric case consultation. This will require preparation, accurate and up-to-date patient registry and records maintenance, and the ability to make case presentations accurately and concisely.
  • Working directly with patients both face-to-face and by phone contacts. You'll contact a vast majority of your patients by phone.
  • Brief therapeutic interventions drawing on evidence-based psychotherapies, psychoeducation, safety planning, assisting family members in understanding and supporting the patient, grief counseling, and referrals to community resources are some of the skills needed. Additional duties as assigned.

Required Qualifications*
  • A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
  • A current license to practice social work in the State of Michigan.
  • LMSW (LLMSW's will not be considered)

Desired Qualifications*
  • Previous experience with medical and mental health social work.
  • Skilled in developing rapport and effectively communicating with a diverse patient population.
  • Efficient, accurate assessment of patient needs and effective case dispositions.
  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.
  • Independent, sound decision-making is necessary for success in this position.
  • Effective collaboration with a large multi-disciplinary team.
  • Proficiency in keeping accurate and timely clinical records by computer.
  • Experience with evidence-based psychotherapeutic interventions (e.g., CBT, PST, ACT, DBT) and familiarity with brief, structured interventions (e.g., Motivational Interviewing, Behavioral Activation) along with a basic knowledge of psychopharmacology.

Work Schedule
This is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00.

Underfill Statement
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification

Background Screening
